H1N1 Swine Flu Vaccination Clinic for West Orange Residents
The West Orange Health Department is holding an H1N1 Swine Flu clinic for West Orange residents 18 years and over on Saturday, January 9, 2010 from 1 pm-4 pm in the Community Room of the John P. Renna, Jr. House at 10 Gaston Street. The H1N1 intranasal vaccine will also be available for healthy people 18-49 years of age.
A second clinic will be held on Saturday, January 23, 2010 at the same time and location for children 6 months through 9 years old receiving their second dose of the H1N1 flu vaccine and for all children up to 18 years of age receiving their first dose. Vaccinations for all West Orange residents 18 and over will be provided as well as the H1N1 intranasal vaccine for healthy people 2-49 years of age. Children under the age of 18 must be accompanied by a parent or guardian.
Please download and complete an H1N1 Influenza Vaccine Consent Form at www.westorange.org and bring it with you on the date of the clinic.
